They are a suitable alternative to dentures for rehabilitation of missing teeth in the entire upper and/or lower arch.

What’s even better is that the implant insertion can be done in one day.

After getting the implants, you can go home with a temporary prosthesis. Return 3-8 months later when the implants have fused completely with the jawbone.

Did you know? 20% of the world’s adult population has complete edentulism (toothlessness).1

How Much are All on Four Implants in Costa Rica?

Dental work in Costa Rica is quite affordable as compared to the United States and Canada.

Procedure Price Duration
All-on-4 Implants fixed full-arch restoration  $13,500 Trip 1: 4 Days

Trip 2: 10 Days

Most patients mistakenly go by the lowest advertised price. Don’t!

More than the low price, it’s important to consider the quality of implants, reputation of the clinic, and credentials of the dental surgeon.

Also, consider the additional services offered.

What Explains the Cost Difference?

This price is much lower than what is charged in the USA and Canada. The following table shows the huge difference in the price:

Country Price of Upper/Lower All on 4
U.S.A $20,000- $30,000
Canada $30,000- $35,000
Costa Rica $13,500

This huge gap is due to the following reasons:

  • US dentists pay very high malpractice insurance
  • Low cost of living in Costa Rica
  • Lower overheads like, rents, utilities, etc. in  Costa Rica
  • Lower salary of dental staff in Costa Rica

Alternatives to All on 4 Dental Implants

All on 6 and 8 are considered to be better alternatives to All on 4 implants by many dentists. See why:

All on 4 implants insertion is a very invasive surgical intervention. Also, in case one of the implants fails, then entire restoration can fail.

Good alternatives to All-on-4 implants in Costa Rica are All on 6 and all on 8 implants. Reasons are:

  • Lower price.
  • Less invasive than all on 4 and implants are placed through a flap-less technique.
  • Unlike all on four where remaining healthy teeth are removed, in all on six/eight, they are restored.
  • Preferred because they allow for achieving greater strength and are more stable in the long run.
  • Provide more support to the jaw which significantly helps improve the bite as compared to All-on-4 implants.
  • Placing additional implants helps in equally dispersing the pressure exerted by a bite or the act of chewing.
  • “All on 6/8″ technique uses fixed bridges built in porcelain fused to noble metal, which gives a natural look to the smile.

The All on 4 technique involves pulling the good teeth out and placing the implants in the jaw and screwing an acrylic denture on top of them.

Our dental clinic in Costa Rica, on the other hand, believes in preserving the natural teeth.

For this purpose, it places six to eight implants depending on the bone quality, which are topped with a porcelain bridge.

What is All-on-4?

An all-on-4 dental implant is a fixed full-arch restoration technique in which four titanium screws are implanted into the jawbone.

These act as roots for replacing the missing teeth. It is an effective solution for full-arch rehabilitation using tilted implants.

The All on 4 implant system was first identified by Nobel Biocare as an effective and pocket-friendly solution for full arch restoration.

Ivory and tooth-shaped shells were used by ancient Egyptians as a replacement for missing teeth.

Characteristics

  • Uses only four implants for rehabilitation of the entire upper or lower jaw, unlike traditional methods which would require six to eight implants.
  • Functions immediately with a fixed provisional bridge.
  • A graft less solution, as the available bone is utilized for implants.
  • Implants towards the back of the jaw are tilted at a 30-45 degree angle.

Procedure

  • Teeth and/or gums are examined by specialized dentists to check if the patient is fit to get all on four. The implant can only be placed, if the patient has adequate bone density.
  • A CT scan is performed so that the placement of the implants can be planned.
  • After the patient is sedated, four implants are placed in the jaw, removing any remaining teeth.
  • These implants are placed through a flap technique, meaning that the dentist opens up the gums, exposing the bone and inserts the implants.
  • Two implants are placed in the front of the mouth while the other two are placed towards the rear of the mouth.
  • The all on four dental implants are secured with a provisional denture which helps to eat and speak normally.
  • Once the teeth and implants heal, a final denture is fitted, which can be either a removable or fully-fixed denture.

Advantages

  • Implants feel more like natural teeth because they are embedded in the bone.
  • They won’t cause bone atrophy or slip-like dentures.
  • This full-arch maxillofacial surgery gives aesthetic results.
  • All on 4 dental implants give you the confidence to smile again.
  • As the implant is made of titanium, which is biocompatible, it fuses with the living bone and functions as a part of it.
  • Usually, bone grafting is not necessary.

The following diagram shows the likelihood of patients to recommend All-on-4 implants and their satisfaction levels:

All-on-4-Satisfaction and Recommendation Rates

All on 4 Implants Success Rate

The all on four procedure has a good track record. When it comes to looks, functioning, speech, and self-confidence2, 95% of patients are satisfied with their new teeth and 98% would recommend this treatment.

The following table shows the survival rates of All on 4s at 3 years, 5 years, and 10 years follow-up:2

Time Period Survival Rate of All on 4
At 3 years 99%
At 5 years in the upper jaw 98%
At 10 years in the lower jaw 94.8%

Since these are immediate load implants, they can be topped by a temporary bridge. This allows immediate functioning.

Renowned dental surgeon, Dr. Paul Malo, was the first to treat a patient with Nobel Biocare All on 4 system in 1998.

Things to Consider Before You Proceed

  • Discuss your restoration goals with the dentist.
  • Understand the benefits and limitations of the procedure.
  • Be aware that the procedure may result in implant failure, although it happens rarely.
