What is All-on-4?

An all-on-4 dental implant is a fixed full-arch restoration technique in which four titanium screws are implanted into the jawbone.

These act as roots for replacing the missing teeth. It is an effective solution for full-arch rehabilitation using tilted implants.

The All on 4 implant system was first identified by Nobel Biocare as an effective and pocket-friendly solution for full arch restoration.

Ivory and tooth-shaped shells were used by ancient Egyptians as a replacement for missing teeth.


  • Uses only four implants for rehabilitation of the entire upper or lower jaw, unlike traditional methods which would require six to eight implants.
  • Functions immediately with a fixed provisional bridge.
  • A graft less solution, as the available bone is utilized for implants.
  • Implants towards the back of the jaw are tilted at a 30-45 degree angle.


  • Teeth and/or gums are examined by specialized dentists to check if the patient is fit to get all on four. The implant can only be placed, if the patient has adequate bone density.
  • A CT scan is performed so that the placement of the implants can be planned.
  • After the patient is sedated, four implants are placed in the jaw, removing any remaining teeth.
  • These implants are placed through a flap technique, meaning that the dentist opens up the gums, exposing the bone and inserts the implants.
  • Two implants are placed in the front of the mouth while the other two are placed towards the rear of the mouth.
  • The all on four dental implants are secured with a provisional denture which helps to eat and speak normally.
  • Once the teeth and implants heal, a final denture is fitted, which can be either a removable or fully-fixed denture.
