International Destinations: India, Mexico, Costa Rica, Jordan, Thailand, Philippines
As per the National Health and Nutrition Examination Survey III, in the United
States, about 57% of adults in the age group of 65 to 74 years, and 51% of
adults in the age group of 55 to 64 years, wear partial or complete dentures.
In most cases, the need to wear dentures is attributed to the loss of teeth
due to gum disease, bacterial infections or periodontal diseases.

Dentures are simple prosthetic devices that are customized for each individual,
to restore the function of the missing teeth, like, the ability to chew, improving
the speech, protection of the gums, and cosmetic appearance. However, most
conventional dentures are removable. These removable dentures require considerable
amount of maintenance - cleaning, washing, etc to avoid secondary infections
of the mouth.
In terms of their ability to restore the functions of the lost teeth, removable
dentures are a poor substitute. Removable dentures provide just 10% of the
chewing ability of the real teeth; they interfere with the ability to speak
and tend to be a cause of public embarrassments if they slip out of their
position. Fixed and removable dentures are types of partial dentures that
are worn by patients who have lost only some of their teeth. With regards
to financial considerations, fixed denture costs are fairly higher than removable
dentures; however going abroad for dental care gives several options for affordable
dentures. With quality of medical care improving across the globe going for
permanent dentures abroad is highly beneficial as the cost of permanent dentures
Abroad is about 50-60% lower than that in the United States.
What are Fixed Dentures?
Fixed denture prosthesis are devices used for dental restoration (or filling),
which are attached permanently to the tooth roots, natural teeth or dental
implants. Fixed dentures (similar to removable dentures) are a type of partial
dentures, which can be used in patients who have lost only few of their teeth.
Fixed full dentures are used when the patient is completely edentulous and
the dental bridges are supported by dental implants.
Fixed partial dentures fill the gaps or spaces caused due to the loss of
one or more teeth, and hence, fixed partial dentures are often referred to
as "dental bridge" as they perform the bridging function. A dental bridge
(i.e. fixed partial denture) is composed of two dental crowns, for teeth on
either side of the gap and the false tooth (s) that lies in between. The fixed
partial dental bridge is supported by natural teeth, dental implant or a combination
of both, and is firmly cemented with the supporting structures and thus not
removable. The dental bridges or fixed dentures are composed of different
types of material, which include porcelain, porcelain mixed with metal, gold.
The cost of fixed dentures depends upon several parameters like the material
to be used, skill and expertise of the technician, whether additional procedure
like filing or root canal would be required for the support, etc. Overall
fixed dentures are more expensive than removable dentures; however fixed dentures
abroad, can save overall expenditure up to 60% to 70%
Fixed dentures Vs Dental implants
Dental implants involve the surgical fixture of the implant into the jaw
bone, which subsequently integrates with the jaw bone to stabilize the root
of the tooth. In fixed dental implants, a screw is inserted into the gum which
works as the fixture, to which the prosthesis is attached to either by cement
or another screw. While selecting the procedure it is essential to understand
the benefits and limitations of dental implants vs. bridges
- Fixed dental bridges require existing tooth on either side (or at least
one side) of the gap. Dental implants don't require existing tooth.
- The exiting tooth is filed to fix the prosthetic bridge material, while
dental implants don't involve any form of filing or reshaping of the existing
tooth. Bridges can't be used for gaps that are too long, unlike dental implants
that can be used to fill any form of gaps.
- With regards to maintaining oral hygiene, dental implants are easier
to care for compared to dental bridges and the frequency of other gum diseases,
caries, etc is fairly lower in cases of dental implants
- Dental implant costs are considerably higher than cost of partial dentures
or dental bridges. Fixed bridges may on an average last for 10-15 years,
while dental implants last throughout the life.
Types of Fixed Dentures
Traditional Bridge: These dental bridges comprise of two crowns for
the supporting natural teeth or dental implants, on either side of the gap
and a false tooth located in the centre. These dental bridges are made up
of ceramic or porcelain fused with metal (PFM). This is the most commonly
used dental bridge
Cantilever Bridge: When supporting teeth are present only on one side
of the gap, this type of dental bridge is used. Two crowns are fused to the
false tooth and used to fix the bridge to the available supporting tooth
Maryland Bonded Bridge: A dental bridge which is made up of plastic
teeth and gums and is supported by a metal framework. This dental bridge is
extremely cheap and simple to apply compared to the other types
Installing fixed dentures: The procedure
- After the preliminary assessment of the oral condition and the location
of anchoring tooth, the dentist would begin by filing the anchoring adjacent
teeth. This procedure is referred to as "fabrication" and the filing is
aimed to reduce the size of the anchoring teeth so as to create room for
the restoration material and also maintain natural alignment and content
with surrounding teeth.
- Once the filing is completed, the dentist takes an impression of the
teeth. This impression is to be used to prepare the dental crown for placement
of the bridge.
- The crowns are made of ceramic, metal or porcelain fused with metal.
- Once the crowns are ready, the false tooth (teeth) is bonded with them.
Now, the dental bridge is ready and is placed into the oral cavity of the
patient
- The entire procedure would require several visits to the dentist, initially
for filing of the anchoring tooth and subsequently for placing the dental
bridge.
- Fixed dentures may last for eight to ten years, depending upon the level
of oral hygiene maintained by the patient. A rigid oral care is required,
which includes regular brushing; flossing and regular visits to the dentist
to control build up of plaque. If adequate care is not taken, bacterial
infection would affect the longitivity of the prosthetic dentures, while
also causing additional damage to other healthy teeth.
Advantages of fixed dentures or dental bridges
Fixed dentures or dental bridges present several advantages which include,
- Dental bridges look like natural teeth and perform most of the functions
of the real teeth with equal effectiveness
- Dental bridges help in proper grinding and chewing of food. The dental
bridges have greater strength and are more robust compared to the removable
dentures
- Fixed dentures offer greater stability as compared to removable dentures.
Fixed dentures significantly reduce discomfort and trauma that is associated
with shifting and moving of removable dentures.
- They restore natural tone and speech.
- Overall, a dental bridge makes a person look younger and thereby restores
a greater level of confidence.
- Compared to dental implants, the permanent dentures cost is fairly lower,
making this procedure more affordable and beneficial to the patient
Risks associated with using Fixed Dentures
There are certain risks that are associated with the use of fixed dentures
which include,
- Since dental bridges or fixed dentures are fitted permanently on to the
gums, it requires a highly skilled professional, with adequate equipment
and infrastructure to take proper measurements and ensure that the prosthesis
is compatible to your oral cavity and face.
- High level of oral hygiene has to be maintained after the procedure with
regular visits for dental check up to avoid further problems.
- The procedure requires certain extent of trimming or filing of natural
adjacent teeth which may weaken the tooth and increase stress on the tooth
due to overload
