Assisted hatching (AH) is a procedure which is used in conjunction
with in vitro fertilization (i.e. for the fertilization of the egg that takes
place outside the human body). The assisted hatching procedure is specifically
meant to make modifications in the wall of the embryo (which is fertilized
using any of the techniques of in
vitro fertilization), so that it gets hatched and subsequently gets implanted
easily into the uterine walls.

The principle concept behind the procedure is to provide assistance/facilitate
the embryo in escaping from the outer shell (i.e. zona pellucida), so that
it can implant in the mothers uterine wall. This procedure of assisted hatching
is useful for women with poor quality of embryo, advanced age or endometriosis,
all of which contribute to a difficulty in implantation of the embryo in the
uterine walls. There is substantial clinical evidence that assisted hatching
tends to improve the chances of implantation, especially among couples using
IVF technology. Assisted hatching technology is available at some of the leading

When Assisted Hatching (AH) is needed?
Assisted hatching is used in concurrence with in vitro fertilization procedures
to improve the chances of implantation of the embryo. This procedure is required
in cases of,
- Advancing age, which, reduces the changes of normal fertilization and
assisted hatching is used when the age of the female partner is above 38
years
- Older women suffer from complications like endometriosis, which further
makes implantation of the embryo difficult
- In some females the quality of the eggs is poor; these cases may require
assisted hatching.
- In some females, the follicle stimulating hormone gets elevated on the
third day after the ovulation, making implantation difficult. Assisted hatching
is required in such cases
- In couples who have tried in vitro fertilization cycles for once or twice
previously, but have failed to conceive, despite otherwise good results.
Success rate with Assisted Hatching Technique:
Clinical evidence has suggested that assisted hatching tends to improve the
outcome of an in vitro fertilization (IVF) treatment, though these outcomes
may vary from clinic to clinic. The pregnancy rates using assisted hatching
technique among women between the ages of 35 to 39 has been found to be almost
50%. With increasing age the pregnancy success rates go down significantly.
Yet, an IVF procedure performed with assisted hatching has a better chance
of conception, than IVF procedure performed without assisted hatching.
Assisted Hatching (AH): The procedure
- Assisted Hatching is a delicate and complex procedure. It requires a trained
and skilled embryologist to perform the procedure. The assisted hatching
is conducted using a machine called the micromanipulator, immediately after
the process of fertilization.
- After the ovum has been fertilized by the sperm, the embryo is placed
in a culture medium in a petri dish.
- The embryologist, then with the help of a special pipette holds the embryo
in its position. Next the embryologist places acidic solution next to the
outer shell of the embryo, using a hollow needle. When this acid solution
comes in contact with the outer shell (i.e. zona pellucida), the protective
layer starts disintegrating creating a small hole. This hole in the zona
pellucida will subsequently help the embryo hatch and implant itself on
the uterine wall.
- Newer technologies include the use of lasers to create a hole in the outer
shell covering of the embryo.
- After the above procedure is completed, the embryo is washed and placed
into the incubator until embryo transfer is done.
- The embryo transfer is done in a short while after the hatching. The
embryo is placed in the women's uterus, with the hope that it will implant
and mature into a full term live birth.
Risks Associated with Assisted Hatching
Assisted Hatching is a simple procedure; however it is associated with certain
complications for both the mother and the child. The risks for the child include,
- Fetal complications
- Physical deformities
- Conjoined twins
The procedure can occasionally cause complications for the mother which include,
- Elevated Levels of Blood pressure ( Hypertension)
- Infections: which occur due to an intake of steroids prior to the procedure,
which in turn suppresses the immune system making it prone to infections
- Gastrointestinal disturbances like nausea and vomiting: Is attributed
to the use of antibiotics to prevent infections
- Mood swings
Limitations of Assisted Hatching:
- Assisted hatching is a complicated procedure which requires great skills
to ensure that the procedure is successful. The couple would have to depend
considerably on the skills of the embryologists, to ensure that the assisted
hatching has been done successfully
- One of the greatest limitations of assisted hatching is the substantial
risk of complications during pregnancy.
- Further, this technique can help only those couples, where the female
has specific problems for which this technique is meant.
